What to Send
- Patient face sheet with contact information.
- Insurance information, including primary and secondary coverage.
- Recommended garments and the body areas being treated.
- Therapist's name, facility, and contact information.
If available, include the signed SWO, relevant clinical notes, and completed measurement forms.
What Happens Next
- We review the referral and verify insurance benefits.
- We coordinate missing documentation with the patient, therapist, and prescribing practitioner.
- We confirm garment details and discuss any expected patient responsibility before ordering.
